Instructions for Forms
The English versions of the forms (courtesy translations) are intended solely to help you fill out the French forms. Do not fill in the translated forms.
To access these forms, click on Translation under the title of each form.
Downloading the forms
All our forms are available in dynamic PDF format and can be completed directly onscreen.
You must use Adobe Reader 7.0 (or later versions), available free of charge from the Adobe website. Please note that the free version of Acrobat Reader, however, does not allow you to save the information entered.
Printing the forms
Please print the forms using black ink on white paper to ensure superior quality when they are microfilmed.
To obtain the required number of copies, it is important to click on the "print" button found on the last page of the form.
Submitting the forms
Since the forms cannot yet be submitted electronically, once they have been duly completed and signed, you must either bring them and the prescribed fees directly to one of the two Services Québec offices that provide services related to the enterprise register, or mail them to:
